Community Giving

SCAN Health Plan Community Giving is focused on helping to meet immediate needs – and close gaps – in community services in the areas where SCAN operates. Giving initiatives may include funding for community-based, nonprofit organizations whose services help older adults meet basic needs (such as nutrition, shelter, health and socialization), and on enhancing an individual’s ability to remain independent in their own community.

SCAN Community Giving Grants

The SCAN Community Giving Committee identifies organizations whose work fits well with SCAN Health Plan program priorities. As a result, all grantees are identified and contacted by SCAN Health Plan directly. Support is provided through one-time operating grants.

SCAN Health Plan Community Giving accepts unsolicited funding inquiries on an on-going basis. To submit a request, please take our eligibility quiz and apply online using the link below.
