The ISNP Embrace Plan is a Medicare Advantage plan specific for residents of long term care communities. It is designed to keep help keep you as healthy as possible without having to leave comfort of your home, and increase communication with family and staff at the community. The Scan Mobile Team, myself and the community all work together to have your medical care brought to you!

If you are a resident of a long term care community and currently have Medicare Part A and Part B you may qualify to have your medical care brought to you at your community.
