SCAN offers a routine vision care benefit through EyeMed® Vision Care for all plans.

Individual plans vary, but in general, the routine vision benefit includes:

  • Yearly routine eye exam
  • Eyewear allowance to spend on frames and lenses or contacts
  • Choice of vision providers from the national EyeMed Select network

Check your Evidence of Coverage to see exactly what your plan includes.

Remember: For medical eye problems, see your primary care doctor.

How a Self-Referral Benefit works:
For your medical care, you will see your primary care doctor first whenever you think you may need specialty care (some SCAN plans vary). Many of the supplemental benefits SCAN offers allow you to self-refer, meaning you don’t need a referral from your primary care doctor first.

Yes, your vision benefits cover either contact lenses or eyeglasses.

For medical eye issues, see your primary care doctor. Your doctor may refer you to a specialist so you can get the medical care you need.

Q: Will I get an EyeMed ID card?

No, just show your SCAN member ID card when visiting a contracted EyeMed doctor.

Q: If my lenses or frames become lost or broken, are replacements included as part of the benefit?

No, replacement lenses or frames are not included as part of your vision benefit.
