2024 Medication Therapy Management (MTM) Program

Our Medication Therapy Management (MTM) program can help you manage your medications and get the best results from all the medications you are taking. A team of pharmacists and doctors developed this program to help ensure that you are using medications that work best to treat your medical conditions and help us identify possible medication errors.

If you have three or more of the following medical conditions, take 8 or more maintenance Part D medications at the same time, and have annual Part D medication costs of $5,330 or more in 2024 (includes amounts paid by both you and the plan), you are likely to be eligible for our program. If you are considered at highest risk of adverse events or overdose related to opioid use and part of our Drug Management Program, you will also be eligible for our program.

The MTM program provides you with a Comprehensive Medication Review (CMR) and Targeted Medication Reviews (TMRs). If you are eligible for this program, you will be notified by mail and then contacted by telephone to review your medications with our pharmacy team. SCAN partners with Arine to provide MTM services to our members so you may receive a call from an Arine team member to complete a comprehensive medication review.

During a CMR, which will take about 30 minutes, a pharmacist will review all of the medications you are currently taking and will:

  • Answer any questions or discuss any concerns you have about your medications
  • Talk about possible cost-saving opportunities and/or lower-cost alternatives for your medications
  • Ensure your medications are safe for you, check for drug interactions, and address side effects
  • Notify your doctor of any drug interactions or side effects

Upon completion of a CMR, you will be mailed a Recommended To-Do List and Personal Medication List that can be taken to your doctor’s appointment for further medication management. Our pharmacy team will also do TMRs every 3 months and may reach out to your doctor by sending recommendations regarding the medications you are currently taking. In addition, you will receive information about proper ways to safely dispose of unwanted, expired, and unused medications that are controlled substances.

This program is not a benefit; therefore, if you qualify for our program based on our criteria, we will automatically enroll you in the program and offer you program services over the phone or send you information by mail. The MTM Program is free of charge to eligible members. Program participation is voluntary. If you decide not to participate, please notify us and we will withdraw your participation from the program.
