Arizona (HMO-POS I-SNP)

SCAN Embrace has evolved into an Open Network Plan! Starting 1/1/2024, we’re transitioning to a Point-of-Service (POS) Medicare Advantage I-SNP plan. This transformation offers you enhanced access to a broader range of services, whether they are within or outside of the network.

SCAN Embrace covers the same services as Medicare and providers will be paid at Medicare rates.

Embrace POS members can see any provider that accepts Medicare.

• Open Network: Point of service (POS) plans allow members to use providers that are in and out of the plans’ designated network. No contract required.
• Authorizations: Members can see providers without authorizations for initial visits and specific outpatient services. For a complete list, visit the provider portal, scan the QR code on the back of your card, or contact SCAN Desert Health Plan at 888-540-7226.
• Copay: SCAN Embrace members who see an out-of-network provider will be charged a $10 copay by the participating provider office (Copay waived if member has AHCCCS/ALTCS).
