How to use your Legal Services Reimbursement Benefit

Available to members of the SCAN Affirm plan.

At SCAN, we understand it’s important to have the right legal documents in place to make sure your wishes are followed if you become unable to make decisions about your care. That’s why we’re here to make it easier to get the documents you need.

With this benefit, SCAN will repay you up to $100 for legal services each year for qualified legal documents, such as a power of attorney for health services (POA) or an advance directive.
