SCAN MyChoice (HMO)
A plan for those who prefer self-service options and easy-to-understand benefits. Includes more online options, a simpler pharmacy network, and valuable extra benefits--along with comprehensive coverage with many $0 copays.
SCAN Balance (HMO C-SNP)
Designed for people with diabetes, cardiovascular disorders and/or chronic heart failure, offering $0 copays for insulin and many brand name diabetes meds, as well as gym benefit, vision care, and more. All this in addition to comprehensive medical and hospital coverage.
SCAN Strive (HMO C-SNP)
Designed for people with one or more chronic conditions. People who qualify for Medicare and full Medicaid coverage may have lower copayments. Includes $0 drugs and $0 doctor visits, as well as vision and dental care, transportation and much more.
