Designed for people with diabetes, offering $0 copays for insulin and many brand name diabetes meds, as well as gym benefit, vision care, and more. All this in addition to comprehensive medical and hospital coverage.
SCAN Strive (HMO C-SNP)
Designed for people with one or more chronic conditions who have Medicare and full Medicaid coverage. Includes $0 drugs and $0 doctor visits, as well as vision and dental care, transportation and much more.
